My Medication List
To provide you the most safe and effective care, it is important that we have a list of all medicines you are taking, including prescriptions, over-the-counter medicine, herbs and vitamins. A doctor or nurse will collect this information from you on admission.
Please do not bring medications with you. Your doctor will make arrangements for you to receive the medications you need while you are here.
The best way to manage your medicines is to keep a current list of everything you are taking. And be sure to update the list every time you start or stop a medication.
