LifeCare Social Service Department provides services in acute care, long term care, home health care and hospice for all ages from infancy through geriatric. The Social Service Department also is responsible for the discharge planning process and coordinates the Swing-bed program.

Other Services Provided by LifeCare Social Workers

LifeCare social workers assess patients, residents and clients of all ages to determine their needs as they are discharged from the hospital or other service. They help with education and coordination of services available. Our social workers follow mandated reporting guidelines in the event of suspected physical/emotional abuse or neglect, sexual abuse or financial exploitation. They make proper referrals or provide appropriate counsel for patients, residents and clients according to their assessed needs. Our social workers coordinate and facilitate patient, resident and client family conferences to assist them in decision making or problem solving. Our social workers assist patients and community members with Health Care Directives. They provide information and assist in making sure the document is properly completed. This service is free of charge to anyone that requests assistance in completing a Health Care Directive. They also provide education to staff regarding patient, resident and client rights, including the Minnesota Vulnerable Adult Act and the Federal Patient Self Determination Act and the Minnesota State Reporting of Maltreatment of Minors Act.
